Septrin Forte

How does it work?
There are two active compounds in co-trimoxazole, trimethoprim and sulfamethoxazole (previously spelt sulphamethoxazole in the UK). Sulfamethoxazole belongs to the sulphonamide group of antibiotics. Trimethoprim and sulfamethoxazole both work inside the bacterial cell, where they stop the manufacture of a substance called folic acid (folate). Folic acid is necessary for the production of genetic material (DNA). Without the production of DNA the bacteria is not able to reproduce, and the spread of the infection is stopped (a bacteriostatic effect). Co-trimoxazole is used to treat a small number of serious infections but serious side effects limit its use.

What is it used for?
·  Chronic bronchitis
·  Ear infections
·  Infection of the bladder or tubes that pass urine
·  Prevention of pneumonia caused by the bacterium pneumocystis carinii (PCP). The risk of this infection is increased in patients with decreased immune system function.
·  Treatment of pneumonia caused by the bacterium pneumocystis carinii (PCP)

Warning!
·  You should ensure that you drink plenty of fluids while receiving treatment with this medicine.
·  People taking this medicine should have regular blood tests.

Use with caution in
·  Asthma
·  Blood disorders
·  Decreased kidney function
·  Decreased liver function
·  Elderly people
·  Infants under 6 weeks of age
·  Lack of the enzyme G6PD in the blood (G6PD deficiency)

Not to be used in
·  Kidney failure
·  Life long inherited blood diseases which can cause a variety of symptoms, including mental health problems (porphyrias)
·  Liver failure

Side effects

·  Inflammation of the lining of the mouth (stomatitis)
·  Damage to the liver
·  Damage to the kidneys

How can this medicine affect other medicines?
Co-trimoxazole increases the risk of kidney damage if this drug is taken with cyclosporin. Co-trimoxazole when given with the anti-malarial pyrimethamine may decrease the levels of folic acid in the blood. Co-trimoxazole when given together with the antiepileptic drug phenytoin may increase the level of phenytoin in the blood and may also enhance the effects of the drug in lowering the levels of folic acid in the blood. Co-trimoxazole increases the risk of developing a disturbed heart rhythm when given with amiodarone. These medicines should not be used together. The blood thinning effects of nicoumalone and warfarin are increased. The effect of sulphonylureas may be enhanced leading to low blood sugar levels. Co-trimoxazole when given together with methotrexate will enhance the antifolate effect of the two drugs and may lead to toxic side effects. Co-trimoxazole may increase the blood levels of the antivirals lamivudine and zalcitabine. The use of high doses of co-trimoxazole should be avoided with lamivudine. Co-trimoxazole may reduce the effectiveness of the combined oral contraceptive pill.
